In two separate studies, Christina Katsakou et el. (2010) and Jelena Jankovic et. al (2011) trace the practical implications of treatment satisfaction and caregiversââ¬â¢ experiences respectively. Both studies cover specific aspects of psychiatric treatment in the UK. The first study focuses on the coercion and treatment satisfaction among patients, who have been admitted for psychiatric treatment involuntarily. The research, conducted in 2010, reveals the impact of coercion on the satisfaction of treatment among patients, as well as the psychological aspects of coercion. The study has been conducted as an observation in 22 hospitals in England, where a total of 778 patients were recruited (Katsakou et. al, 2010). Their satisfaction with the treatment they received has been measured at different stages: one week, one month, three months and one year after the admission. In order to measure the levels of satisfaction, the authors have used factors such as clinical improvement and clinical characteristics. The results from this study were obtained using standard statistical analysis, and indicated an increase in the satisfaction among involuntarily admitted patients between their first admission and the different follow ups. The second study, conduc ted in 2011, focuses on the experiences of family caregivers during involuntary hospital admissions of their relatives. It is a qualitative study, which used as a research method semi-structured interviews, conducted with 29 caregivers whose relatives have been admitted involuntary in 12 hospitals across England (Jankovic et.al., 2011). Throughout the study, major themes have been identified, such as relief and conflicting emotions, frustration with the delay of getting help, etc. The results of the second survey have concluded that the role of the family caregivers can be enhanced if their duties are valued enough, without turning into a burden. The purpose of this brief academic review is to critically compare both studies, highlighting their strengths, weaknesses and possible contributions to the literature on the subject. Both studies provide valuable insight on the subject of treatment of patients with mental illnesses, and reveal the interactive nature of the clinical process as a dynamic interaction between different elements ââ¬â institutions, caregivers, and patients. Both studies manage to reveal the intricacy of the connection, which exists between coercion and satisfaction in the first case, and family caregivers as active elements in the process of involuntary admission in the second case. The first study uses a quantitative research method and statistical analysis, based on an observational study. The study has been conducted in 22 hospitals in England. The advantage of the choice of this method for the purposes of study is its accuracy and straightforwardness. Results obtained through observation are easier to analyse, and presented in a comprehensible and consistent manner. In terms of the design of the research, the use of timeline base is a feasible option, which meets the research aims of the study and unfolds different aspects of satisfaction among patients. The fact that the patients have been examined at three different periods following their first admission provides the researchers with the possibility to explore how satisfaction (or dissatisfaction) develops gradually. This is an important feature of the research design deployed, because here satisfaction is discussed in relation to memory and emotions, which change over time. This changeability has been ca ptured with the choice of the design. Also, it is a good way to operationalize and thus measure the main variable ââ¬â patientsââ¬â¢ satisfaction. Despite the fact that the operationalization of satisfaction was enhanced through the use of specific research design, the study could have also benefited from a bigger emphasis on open interviews, conducted with a certain (perhaps smaller) portion of the patients. This is because interviews allow for more abstract and personal issues to be uncovered in the research and these are issues which are usually unquantifiable and difficult to detect in observational studies and semi-structured interviews. As far as data collection is concerned, the authors of the first study have used two different models, designed to measure satisfactionà one concentrating on potential base lines predictions combining satisfaction scores from all time points (baselines, one month and three months) and another one concentrating on the results from the follow ups only. The data was analysed using a three step model, and applying standard linear statistical analysis (Katskakou et.al, 2010:287-288). This comprehensive choice of methods and models for data collection has enhanced linear results, which allow readers to obtain an extensive view of satisfaction not only as an isolated variable, but also as a process, which is happening over time. However, one of the weaknesses of this study in the methodological part is the sampling. The researchers have attempted to obtain a representative sample, covering hospitals from different geographic areas and patients of different ethnic and social backgrounds. Little, if anything is mentioned however on how the eligibility of the patients has been identified (eligibility criteria). Another weakness in the methodology part is related to the decreasing number of patients interviewed at the baseline, the first month and the third month and one year. For the baseline, the patients are 778, and for the one year follow up their number has decreased almost in double ââ¬â 396 (Katskakou et.al., 2010: 289). à This might pose some problems related with the generalizability of the results obtained and the consistency of observations. Although it would be a formidable task to keep the number of patients at each point exactly the same, at least proximity in the numbers of interviewees could have been targeted. Another possible weakness of the study is related with the lack of causality between coercion and satisfaction. In other words, the study does not necessarily reveal a cause-effect relationship between the two, because coercion in this ob servation has been explored as an individual projection. However, this can also be looked at as an advantage, because a cause-effect relationship between two abstract concepts can oversimplify their existence in a particular setting. To compare, the second study uses a very different methodology. It is a qualitative study, and the variables measured here are even more abstract compared to the first study. The psychological aspects of personal experiences relating to care are difficult to capture and quantify, and this is important to mention in the methodological review of the second study. In terms of choice of methods, the authors have used semi-structured interviews, conducted among family caregivers of 29 patients admitted involuntary against 12 hospitals in England (Jankovic et.al., 2011: 1). Compared to the first study, here the sample is much smaller. It is arguable whether such a small sample can provide results, which are generalizable. Perhaps the authors have decided to choose smaller number of participants in order to observe the matter more closely. Here it is important to note that the issue of carersââ¬â¢ experiences is sensitive and often a stressful one. Therefore a smaller sample would give t he chance to conduct more detailed interviews, and thus capturing nuances of the matter, which remain unexamined in studies involving larger samples, due to time constraints. Another problem with the sample, just like in the first study, is its ability to represent the population. A closer look at the participantââ¬â¢s characteristics in the second study reveals that in more than 50 percent of the cases, the relationship of the carer to the patient is ââ¬Å"parentâ⬠(Jankovic et.al., 2011: 3). This fact could have influenced the results, since parents tend to be much more concerned for their children. They are concerned first as patients, and then as carers therefore a more representative selection of the carers could have taken place (for example equal number of carers who are patients, partners, siblings or children). Yet, the study manages to make good use of thematic analysis, clustering answers of the patients and identifying four important themes ââ¬â relief and conflicting emotions in response to the admission, frustration with the delay in getting help, being given the burden of care by services and difficulties with confidentiality (Jankovic et.al, 2011:3-4). Just like the first study, the methodology is well-implemented in terms of coding. In the second study, two independent researchers have been selected to code the interviews, and the results have been finalized through a joint discussion (Jankovic et.al, 2011:3). In both studies, the methods chosen have met the research criteria, and have been meticulously implemented to produce comprehensive and well-themed results. Also, the proposed hypotheses have been well tested. Here it is important to note that both studies tackle issues, which are not easily quantifiable or measurable. The first study concentrates on satisfaction among patients which have been involuntary admitted for treatment, while the other one focuses on an even more sensitive and abstract issue, related with personal experiences among family caregivers in the cases when patients have been admitted for treatment. Therefore the authors of both studies have made significant effort in the planning of the research, its design and implementation in order to make the themes of their research measurable. As a result both studies have managed to create consistent results. The first makes coercion and satisfaction measurable, with the implementation of a 0 to 5 scale of coercion and inco rporating the results in a separate model. Critics would suggest that the method implemented in the first study is too rigid for the investigation of issues, which are deeply psychological and reflect the personal perceptions of patients on the way they have been treated. Although the study could have benefited from a combination of qualitative and quantitative methods, its reliance on quantitative techniques only does not affect the overall validity of the results. Perhaps one of the biggest strengths of this study is that through the interpretation of the results, the authors manage to reveal the connection between patientsââ¬â¢ satisfaction and coercion as an individual perception. As already mentioned, the second study relies solely on a qualitative technique. They authors capture the nuances in the experiences of the caregivers, and interpret the results closely adhering to the themes, identified by them during the data analysis stage. Both studies deal with aspects of psychiatric treatment, which are challenging due to their specificity. Therefore they both make significant contribution to the literature and theory on the subject. The first study sheds light upon the complexity of coercion as an individual perception, and its results resonate with those obtained in earlier studies (Lidz et.al, 1998; Sorgard, 2004). Observing coercion as an individual perception, projected by patients as a result of hospital surroundings and treatment, shows a major transition in clinical psychology, and a shift towards a more constructive approach for understanding patientsââ¬â¢ reactions. In this sense, this study can be classified as a constructivist study, because it measures how perceptions are formed and exemplified by particular patients in particular environments. It deviates from earlier studies on the subject, like the ones conducted by Svensson et al. (1994) and Spenseley (1980), which observe patientsââ¬â¢ satisfa ction with treatment in their entirely empirical dimensions, ignoring individual projections. Similarly, the findings and conclusions from the second study (Jankovic et.al, 2011) resonate with conclusions from previous research on the subject (Simson et. al, 2002; Jones et.al, 2009). Therefore this study belongs to a particular body of literature in clinical psychology, which explores the psychological and social impact on families of care for people with mental disorders. Both studies contribute to their relative subjects, and might have important implications in terms of policy reform in health care services for people with mental illnesses in the UK. Recommendations for policy-makers in this sector, stemming from Jankovicââ¬â¢s study include an improved service, which would ensure that carers obtain proper assistance and cooperation from hospitals prior to the admission of mentally unwell relatives. This would have positive implications to the quality of treatment of mentally unwell patients, by enhancing cooperation between carers and institutions, which would inevitably lead to a better distribution of responsibilities. Katsakouââ¬â¢s study might have policy implications as well, because it reveals the necessity for interventions, which would reduce patientsââ¬â¢ perceived coercion. In conclusion, both studies discussed in this review present specific aspects of treatment of mentally unwell patients. Despite some weaknesses in sampling, and some minor limitations, both studies make significant contributions in their relative fields, and offer new, and well-supported angles of interpretation on the themes they cover. Both studies make good use of research methods, despite the differences in the number of participants for the first and the second study. Some issues related to generalizability of the results have arisen, such as the decreasing number of observed patients in the first study and the small sample in the second study. Still, the data analysis and the interpretation of the results obtained have been meticulously carried out and well situated in the context of existing literature. In addition, both studies might serve as a basis for policy-reform in the UK healthcare system, ultimately leading to improvement of the latter. John Steinbecks ââ¬ÅThe Chrysanthemumsââ¬Â Essay -- Steinbeck Chrysanthemum
John Steinbeck's ââ¬Å"The Chrysanthemumsâ⬠John Steinbeck's "The Chrysanthemums" shows the true feelings of the main character, Elisa Allen, through the use of setting and her interactions with other characters in the story. By way of vivid descriptions, Elisa's feelings of dissatisfaction over the lack of excitement in her life are portrayed. Her role as a mere housewife and then the subsequent change to feelings of a self-assured woman are clearly seen. These inner feelings are most apparent with the portrayal of Elisa working in the garden with the chrysanthemums, the conversation she has with the man passing through, and finally, when she and her husband are going out to dinner. Steinbeck's strong and somewhat manly description of Elisa while working in the garden, gives the distinct impression that she is not as weak as a stereotypical housewife would be. He writes that "Her face was lean and strong and her eyes were as clear as water. Her figure looked blocked and heavy in her gardening costume, a man's black hat pulled low down over her eyes, clodhopper shoes, a figured print dress almost completely covered by a big corduroy apron with four big pockets to hold the snips, the trowel and scratcher, the seeds and the knife she worked with." As evidenced by this excerpt you can see that she has covered up her hair with a "man's hat" and has thrown an apron over her dress in attempts to cover up her femininity. This apron also takes on a similar role as a man's tool belt as he works the land. This initial description lends to the notion that Elisa wants to be stronger as a woman, to stand beyond the stereotype of a housewife. Other phrases used by Steinbeck fu rther the above points. The author mentions that her face was "handsome," her work with the scissors was "over-powerful," and her fingers "destroyed such pests." This description shows an inner strength that Elisa possesses; yet she is unable to bring this out. She also hints to the reader that she would like to take on more masculine responsibilities after her husband Henry comments on the size and beauty of her chrysanthemums. He says that he wishes she'd work out in the orchard and "raise some apples that big." She reacts to this by saying "Maybe I could do it too. I've got a gift with things, all right. My mother had it. She could stick anything in the ground and make it grow.â⬠All of these de... ...sion of Steinbeck's short story, Steinbeck has her fall right back into the rut she so despised. She comes back to reality and "turned up her coat collar so he could not see that she was crying weakly-like an old woman." à à à à à Elisaââ¬â¢s inner feelings are most apparent with the portrayal of her working in the garden, the conversation she has with the ââ¬Å"Fixer-Guyâ⬠, and finally, when she and her husband are going out to dinner. Steinbeck offers an array of different details concerning the character of Elisa Allen. The main detail being that she is a strong woman on the inside although she seems to struggle in showing it throughout the story. Because of the "Pot Fixer," she is able to act, although for a short time, like that strong woman. Steinbeck unfortunately brings Elisa full-circle, back to where she started. She notices on the way to dinner her chrysanthemum sprouts at the roadside. This, along with her husbandââ¬â¢s hesitation to allow her curiosity, puts her back where she started, canceling her emergence. One must ask after reading this short story if Elisa will continue this discontented lifestyle. Or will she be able to blossom beautifully for good, as do her chrysanthemums?

Problem: Increase marketing of Chiropractic Services. My aim is to bring strategies that will increase the profitability of your practice, while at the same time improving patient satisfaction and results. In the process of achieving these aims, I also seek to improve the publicââ¬â¢s image of chiropractic care. First Recommendation: To increase profits and public awareness. Rational: Increasing profit is the main goal in any business. To help you increase profits I wish to provide advice in the following areas, as suitable: â⬠¢ debt reduction â⬠¢ retirement planning â⬠¢ overhead reduction â⬠¢ investment strategies â⬠¢ bankruptcy guidance â⬠¢ collection assistance I feel that by using sound business advice in these areas you will improve your profits marginally. Together we will evaluate what products or services are making you money. We will decide whether other products or services should be promoted in addition or in place of current services. To cite a classic business planning question, ââ¬Å"Are you making 80 percent of your profits from 20 percent of your offerings?â⬠Are you wasting time on products and services that donââ¬â¢t return the favor by providing you with profits? Again, together we will find these answers. Why is this recommendation important now? I feel will that with the current economic situation, now is the time to work at increases your profits to its fullest. Now is not the time to let your business take a down fall. Priority ranking of the recommendation: The priority for profits is an important need in business. Proposal: I propose that within the next few years your Chiropractic office will increase profits by increasing services and lowering costs. This will beà achieved by first looking at business expenses. One seriously important area to examine is staffing. This is typically one of a companyââ¬â¢s biggest expenses. You need to ask yourself ââ¬Å"Did you make necessary cuts during the recession or are you still overstaffed? Are you getting your dollarsââ¬â¢ worth from every employee?â⬠Make necessary adjustments now. Next, we need to examine your other expenses as well. These expenses include expense accounts to seminars, up to magazine subscriptions. We will go over your entire profit and loss statements for this year. Remember to track everything. Scrutinize your cash flow, profits and trends every week. Whatââ¬â¢s more, figure out what your companyââ¬â¢s important numbers are. Second Recommendation: Increase marketing and public awareness. Rational: I do not recommend door-to-door surveys or other unsuccessful marketing gimmicks. Nor do I support high patient fees or unwarranted patient visits. Instead, I want to custom-tailor solutions to your distinctive needs and goals. Some marketing tools that I recommend you use are questionnaires, patient fact sheets and health concerns hand outs. These items can be posted in your office, made into brochures, newsletters, newspaper articles, and reading material in specific areas of your office like reception area. Why is this recommendation important now? In order to make money and have a profit you must advertise. This advertising will increase the public awareness of your business and the services you offer. Priority ranking of the recommendation: I feel that this is one of the most important recommendations. Good marketing is the key to great business and success. Proposal: I propose that you step up your marketing by using public relations, Web marketing, email newsletters, and other targeted or large outreach. Lieutenant General John Burgoyne in the American Revolution
General John Burgoyne was a noted 18th century British Army officer who is best remembered for his defeat at the Battle of Saratoga in 1777. First seeing service during the War of the Austrian Succession, he later earned fame as a cavalry officer and leader during the Seven Years War. In this period, he formed his own cavalry unit and commanded troops in Portugal. With the beginning of the American Revolution in 1775, Burgoyne was one of several officers dispatched to Boston. Seeing little opportunity in the post, Burgoyne departed and returned to North America the following year with reinforcements for Canada. While there, he conceived of the idea for what would become the Saratoga Campaign. Given permission to move forward in 1777, his army was ultimately blocked, defeated, and captured by American forces. Paroled, Burgoyne returned to Britain in disgrace. There is some thought that the young Burgoyne may have been the illegitimate son of Lord Bingley. Burgoynes godfather, Bingley specified in his will that the young man should receive his estate if his daughters failed to produce any male heirs. Beginning in 1733, Burgoyne began attending the Westminster School in London. While there, he befriended Thomas Gage and James Smith-Stanley, Lord Strange. In August 1737, Burgoyne entered the British Army by purchasing a commission in the Horse Guards. Early Career Based in London, Burgoyne became known for his fashionable uniforms and earned the nickname Gentleman Johnny. A known gambler, Burgoyne sold his commission in 1741. Four years later, with Britain involved in the War of the Austrian Succession, Burgoyne returned to the army by obtaining cornets commission in the 1st Royal Dragoons. As the commission was newly created, he was not required to pay for it. Promoted to lieutenant later that year, he took part in the Battle of Fontenoy that May and made repeated charges with his regiment. In 1747, Burgoyne pulled together sufficient funds to purchase a captaincy. Elopement With the wars end in 1748, Burgoyne began courting Stranges sister, Charlotte Stanley. After his proposal of marriage was blocked by Charlottes father, Lord Derby, the couple elected to elope in April 1751. This action infuriated Derby who was a prominent politician and he cut off his daughters financial support. Lacking active service, Burgoyne sold his commission for à £2,600 and the couple began traveling around Europe. Spending extensive time in France and Italy, he became friends with the Duc de Choiseul who would later oversee French policy during the Seven Years War. Additionally, while in Rome, Burgoyne has his portrait painted by famed Scottish artist Allan Ramsay.à Following the birth of their only child, Charlotte Elizabeth, the couple elected to return to Britain. Arriving in 1755, Strange interceded on their behalf and the couple reconciled with Lord Derby. Using his influence, Derby aided Burgoyne in obtaining a captaincy in the 11th Dragoons in June 1756. Two years later he moved to the Coldstream Guards and ultimately achieved the rank of lieutenant colonel. With the Seven Years War raging, Burgoyne took part in the June 1758 raid on St. Malo. Landing in France, his men remained for several days while British forces burned French shipping. 16th Dragoons Later that year, Burgoyne went ashore during Captain Richard Howes raid on Cherbourg. This saw British forces land and successfully storm the town. A proponent of light cavalry, Burgoyne was appointed to command the 16th Dragoons, one of two new light regiments, in 1759. Rather than delegate recruiting duties, he directly oversaw the construction of his unit and personally courted the landed gentry in Northamptonshire to become officers or encourage others to enlist. To entice potential recruits, Burgoyne advertised that his men would have the finest horses, uniforms, and equipment. A popular commander, Burgoyne encouraged his officers to mix with their troops and desired his enlisted men to be free thinking in battle. This approach was enshrined in a revolutionary code of conduct he wrote for the regiment. Additionally, Burgoyne encouraged his officers to take time each day to read and encouraged them to learn French as the best military texts were in that language. Portugal In 1761, Burgoyne was elected to Parliament representing Midhurst. A year later, he was dispatched to Portugal with the rank of brigadier general. Following the loss of Almeida to the Spanish, Burgoyne boosted Allied moral and earned fame for his capture of Valencia de Alcà ¡ntara. That October, he again triumphed when he defeated the Spanish at the Battle of Vila Velha. In the course of the fighting, Burgoyne directed Lieutenant Colonel Charles Lee to attack a Spanish artillery position which was successfully captured. In recognition of his service, Burgoyne received a diamond ring from the King of Portugal and later had his portrait painted by Sir Joshua Reynolds. With the end of the war, Burgoyne returned to Britain and in 1768 was again elected to Parliament. An effective politician, he was named the governor of Fort William, Scotland in 1769. Outspoken in Parliament, he became concerned about Indian affairs and regularly attacked Robert Clive as well as corruption in the East India Company. His efforts ultimately led to the passage of the Regulating Act of 1773 which worked to reform the companys management. Promoted to major general, Burgoyne wrote plays and verse in his spare time. In 1774, his play The Maid of the Oaks was staged at the Drury Lane Theater. American Revolution With the beginning of the American Revolution in April 1775, Burgoyne was dispatched to Boston along with Major Generals William Howe and Henry Clinton. Though he did not take part in the Battle of Bunker Hill, he was present at the Siege of Boston. Feeling the assignment lacked opportunity, he elected to return home in November 1775. The following spring, Burgoyne led the British reinforcements which arrived in Quebec. Serving under Governor Sir Guy Carleton, Burgoyne aided in driving American forces from Canada. Critical of Carletons cautiousness after the Battle of Valcour Island, Burgoyne sailed for Britain. Arriving, he began lobbying Lord George Germain, Secretary of State for the Colonies, to approve his campaign plans for 1777. These called for a large British army to advance south from Lake Champlain to capture Albany. This would be supported by a smaller force approaching from the west via the Mohawk Valley. The final element would see Howe advance north up the Hudson River from New York. Planning for 1777 The cumulative effect of the campaign would be to sever New England from the rest of the American Colonies. This plan was approved by Germain in early 1777 despite word from Howe that he intended to march against Philadelphia that year. Confusion exists as to when Germain informed Burgoyne that participation by British forces in New York City would be limited at best. As Clinton had been defeated at Charleston, SC in June 1776, Burgoyne was able to secure command of the northern invasion force. Arriving in Canada on May 6, 1777, he assembled an army of over 7,000 men. The Saratoga Campaign Initially delayed by transport issues, Burgoynes army did not begin moving up Lake Champlain until late June. As his forces advanced on the lake, Colonel Barry St. Legers command moved west to execute the thrust through the Mohawk Valley. Believing the campaign would be simple, Burgoyne was soon dismayed when few Native Americans and Loyalists joined his forces. Arriving at Fort Ticonderoga in early July, he quickly compelled Major General Arthur St. Clair to abandon the post. Sending troops in pursuit of the Americans, they defeated part of St. Clairs forces at Hubbardton on July 7. Regrouping, Burgoyne pushed south towards Forts Anne and Edward. His advance was slowed by American forces which felled trees and burned bridges along the route. In mid-July, Burgoyne received word from Howe that he intended to sail for Philadelphia and would not be coming north. This bad news was compounded by a rapidly worsening supply situation as the army lacked sufficient transport that could traverse the regions rough roads. In mid-August, Burgoyne dispatched a force of Hessians on a foraging mission. Meeting American troops, they were badly defeated at Bennington on August 16. The defeat bolstered American morale and caused many of Burgoynes Native Americans to leave. On September 13, his army began crossing the Hudson just north of Saratoga. Pushing south, it soon encountered American forces led by Major General Horatio Gates which had entrenched on Bemis Heights. On September 19, American forces led by Major General Benedict Arnold and Colonel Daniel Morgan defeated Burgoynes men at Freemans Farm. With their supply situation critical, many of the British commanders recommended a retreat. Unwilling to fall back, Burgoyne again attacked on October 7. Defeated at Bemis Heights, the British withdrew to their camp. In the wake of the action, American forces surrounded Burgoynes position. Unable to break out, he surrendered on October 17. Later Career Paroled, Burgoyne returned to Britain in disgrace. Attacked by the government for his failures, he attempted to reverse the accusations by blaming Germain for failing to order Howe to support his campaign. Unable to obtain a court martial to clear his name, Burgoyne changed political allegiances from the Tories to the Whigs. With the Whig ascent to power in 1782, he returned to favor and served as commander in chief in Ireland and a privy councillor. Leaving government a year later, he effectively retired and focused on literary pursuits. Burgoyne died suddenly at his Mayfair home on June 3, 1792. He was buried at Westminster Abbey.
